Google's SymptomAI Outperforms Physicians in Largest Real-World Test of Conversational Diagnostic AI

Deployed inside the Fitbit app and tested on 13,917 real patients, the Gemini-based system hit 73% top-5 diagnostic accuracy versus 60% for physicians reviewing the same conversations.

Google Research has published results of "SymptomAI," a conversational diagnostic AI built on Gemini 2.0 Flash and deployed inside the Fitbit app, showing the system outperformed physicians who reviewed the same patient conversations. The study — billed as the largest real-world evaluation of conversational diagnostic AI to date — covered 13,917 real patients between June 2025 and April 2026.

Unlike most AI benchmark studies, which rely on written clinical vignettes, SymptomAI actively interviews real patients about their symptoms and ranks candidate diagnoses. That active questioning mattered: it improved performance 27.34% over a passive baseline, according to the research team led by Joseph Breda, Jake Sunshine, and Daniel McDuff. Top-1 accuracy came in at 39.74%.

The caveats that matter

SymptomAI remains a research prototype. The results were released as an arXiv preprint and Google Research blog post — not yet peer-reviewed in a journal — and the system is not available to consumers and holds no FDA clearance. Clinical claims should be read accordingly.

Still, the study is a signal of where consumer health platforms are heading: diagnostic-grade conversational AI embedded directly in the wearable apps hundreds of millions of people already use. With the FDA simultaneously piloting new pathways for digital health technologies, the distance between "research prototype" and "regulated product" may be shorter than it once was.
